upport Coordination is an NDIA support that helps participants build a network of informal, mainstream and funded supports to realise their goals, and live a connected life within their community.

When Support Coordination is on a participant’s NDIS Plan, the participant selects their preferred Support Coordinator. The Support Coordinator then helps the participant connect to other supports and manages the relationships as service delivery progresses.

Support Coordination

The Support Coordinator manages support coordination services for the primary person and their family/carer. The Support Coordinator can assist the primary person in gaining access to programs and support services which may include health, social, education, and other related services.

Specialist Support Coordination is support coordination that is provided by a professional with an Allied health background or someone with Psychological expertise. This is funded for complex participants who have exceptionally high needs or are in a crisis situation.

Specialist Support coordination is time-limited and involves identifying, facilitating, and monitoring supports for primary persons who may face multiple complex high-risk challenges in their support environment. Specialist Support Coordination provides a person-centered approach that aims to reduce barriers and complexity while assisting the primary person to connect with supports and build capacity and resilience.

Our Support Coordinators can 

• Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports
• Strengthen and enhance the primary persons abilities to coordinate supports and participate in the community
• Ensure mainstream services meet their obligations (i.e. housing, education, justice, health)
• Build the capacity of the primary person to achieve greater independence to self-direct services and supports in the long term
• Provide the NDIA with reports on outcomes and success indicators within the agreed reported period
• Identify barriers to access for people in the target groups and act to minimize these barriers where possible
